In my book, Crisis of Abundance, I examined each of these four possibilities. I concluded that the most important factor is (D), and that the only way to slow the growth of health care spending in this country is to change our cultural attitudes about what constitutes good medicine. I recently acquired Crisis of Abundance, and intend to read it, you know, eventually.
